Simpcw membership numbers
As of August 2023:
The Simpcw Membership office is able to process applications for Certificate of Indian Status (CIS). When applying for a Status Card, individuals are required to provide two pieces of I.D. The list of allowable forms of identification are below. Please note that one of the IDs provided MUST have a digitized photo, or be accompanied by a Guarantor’s Form.
Acceptable forms of IDs
Primary ID
- Canadian Passport
Secondary ID
- Birth Certificate
- Marriage or Divorce Certificate
- Provincial Health Card
- Provincial ID Card
- Driver’s License
- Employee ID with digitized photo
- Student ID with digitized photo
- Firearms license
- Original paper Birth Certificate
Restrictions on ID Types provided
Only original documents are accepted – no photocopies are allowed.
Social Insurance (SIN) Cards are not acceptable.
ID must be intact and not expired.
If you are submitting a Guarantor’s Form, you must supply the original form (not a faxed or photocopied version) and the signature of the Guarantor must appear on the applicant’s photograph.
Fees
There is no fee for Simpcw members to receive their cards, unless the card is missing or lost, card then is a fee of $ 25.
For non- Simpcw members, there is a $50 charge. In the case of a missing or lost card.
